CAIS aims to work with the resident to sustain and build on positive change. CAIS, therefore, has established partnership arrangements with a local Housing Association which is may be able to provide high standard accommodation. Together they have a range of move-on facilities available and Ty'n Rodyn residents may be referred to these places subject to availability and suitability.
Residents, who move on from Ty'n Rodyn into a CAIS move-on maintain contact with CAIS Community Services through the support of a support worker who works with them towards the achievement of the goals in their Care Plan.
The referring agents are kept continually informed of progress whilst their client is resident at Ty'n Rodyn. This information will make reference to moving out of Ty'n Rodyn so that appropriate continuing care can be arranged.
When residents leave prematurely in an unplanned way, or are requested to leave as part of the relapse procedure, the referring agents are informed immediately and are expected to fund the cost of returning the client to their home area.
