Ynys Mon Pipeline Project
Overview of the service
The primary purpose of the Ynys Mon Floating Support Scheme is to provide low level housing related support for eligible service users, in different types of accommodation, covering all types of occupancy, in line with SPRG grant conditions
Aims and Objectives
- To enable service users to obtain and maintain independent accommodation by assisting to develop or maintain the necessary skills and confidence to live independently and through this, improve the quality of their lives.
- To ensure that service users gain the outcomes detailed within the SPRG Service Specification.
- To provide low level support that is focused on supporting service users to maintain an existing tenancy which is at risk, due to non-compliance with the terms of the tenancy agreement.
- To support those people who have been offered a prospective tenancy subject to accepting a support package which will equip them with skills to enable them to comply with the terms and conditions of the tenancy agreement.
- To afford access to relevant community facilities agencies/services in support of the individual, to promote independence and social re-integration & inclusion.
- To identify and support individuals in addressing other needs/issues pertinent to enable people to become increasingly independent and develop day to day living skills for independent living.
- To ensure that services are delivered and reviewed on the basis of appropriate support planning, which should include processes for ide3ntifiyinbg the needs and aspirations of service users, agreed outcomes and action plans and review mechanisms.
- To work with the service user to ensure that provision is consistent with and included within, the management plans of statutory and treatment agencies the service users is working with.
